If your provider would prefer to complete a verification form rather than using our documentation guidelines to generate a letter, or  they can’t provide you with a copy of an evaluation, they may complete this verification form.

You do not need to use this form, if you have other supporting documentation to share with us (IEP, medical reports, psychological evaluations, for example.)
